These are the Philosophy Department specific forms which use DocuSign. Please use your UCSB Net ID and password to log in to sign the DocuSign forms

• To enroll in any of these courses, students must for reach out and gain the approval of a Philosophy faculty they'd like to work with.

  • Please note that all PHIL forms MUST be approved by a Philosophy faculty member before the undergraduate advisor can process them.

  • For students who would are interested either in pursuing the Philosophy Department's Distinction in the Major (also known as the PHIL Honors Program) or for students searching for more in-depth research. Also looks great when applying to graduate programs!

  • For students who would are interested either in pursuing the Philosophy Department's Distinction in the Major (also known as the PHIL Honors Program) or for students searching for more in-depth research specifically in Ethics. Also looks great when applying to graduate programs!

• Philosophy Honors Contract: [FOR VIEW ONLY - PDF]

  • Required for students who would are interested either in pursuing the Philosophy Department's Distinction in the Major (also known as the PHIL Honors Program) and/or for students searching for more in-depth research.
  • Note that this for is for any UD PHIL course you are taking - with additional work assigned. No add code is needed as this is any PHIL course numbered 100 - 189.

These petitions are all College of Letters & Science or Registrar petitions. If you are in the College of Engineering or CCS, you may need a different petition.

• Change of Major Petition / Double Major Petition 

  • To switch major(s) to another, to drop major(s), to add an additional major, or to change your declared major(s). This process is through the College of Letters & Science. If you are in the College of Engineering or College of Creative Studies, please contact either the Philosophy undergraduate advisor or a College advisor for the correct petition. 

• Late Add Petition

  • Please contact the course instructor and secure their approval before completing this form. While instructors do not need to sign the form, the College of Letters & Science requires instructor approval. 

• Incomplete Grade Request

  • Contact instructors for approval before completing this form! Please also note that the instructor's UCSB email MUST be used, only their @ucsb.edu email will work on the form. Contact ug_advisor@philosophy.ucsb.edu with any questions.

• Add/Drop/Grading Option Change

  • Contact instructors for approval before completing this form! Please also note that the instructor's UCSB email MUST be used, only their @ucsb.edu email will work on the form.
